Appendectomy in a Patient with PTA Deficiency

Abstract
|

|
A case of PTA (Plasma thromboplastin antecedent) deficiency in a 24 year-old Korean soldier was operated for acute appendicitis in the department of general surgery, 1st Army Hospital.
The main points to be emphasized with regard to this case were the following:
1) Although routine bleeding and coagulation times without additional tests failed to make a diagnosis of bleeder, accurate past history and physical examination were by far the best screening procedures.
2) Petrolatum gauze packings, fresh blool and fresh plasma were used effectively postoperatively for the purpose of local hemostais and deficient factor replacement therapy. In addition, episolone aminocaproic acid was used with questionable results.
3) There were many postoperative complications such assubcutaneous hematoma, wound infection, fecal fistula, retroperitoneal hematoma, hip joint stiffness, hematoma in thigh, renal stone, bedsores, and left peroneal nerve paralysis.
4) In a hemophiliac, postoperative early ambulation and physiotherapy should be carefully administered so as not to start the bleedings again. However. if ambulation and physiotherapy are not begun early enough, complications due to bed rest will occur.
